Passport Acceptance Services

The Akron-Summit County Public Library makes it easier to plan your next trip! Passport Services (including passport photos) are available at the following locations:

Main Library: 330.643.9145
Nordonia Hills Branch Library: 330.467.8595

Apply for a Passport

Bring with you to your appointment:

  1. A completed, unsigned passport application (Form DS-11) filled out in black ink. Form DS-11 and all other passport applications are available on the U.S. Department of State website, TechZone@Main, or at the Nordonia Hills Branch Library.
  2. Proof of U.S. Citizenship (one of the following):
    • Certified U.S. Birth Certificate
    • Previous U.S. Passport (may be expired, must be undamaged)
    • Consular Report of Birth Abroad
    • Certificate of Naturalization/Citizenship
  3. Proof of Identity
    • Valid Driver's License (plus second ID if issued outside of Ohio)
    • Undamaged U.S. Passport (if issued less than 15 years ago)
    • Certificate of Naturalization
    • Valid government ID (city, state, or federal)
    • Valid Military ID
  4. A Passport photo (available at Library during your appointment for $12 fee)

Please note: there are special requirements for children under age 16 and minors age 16 and 17. Refer to the U.S. Department of State website for full information on applying for a passport.

Fees

Check or money order payable to U.S. Department of State per application:

  • Adult passport book - $110
  • Minor (under age 16) passport book - $80
  • Adult passport card - $30
  • Minor (under age 16) passport card - $15
  • Expedited service - $60
  • Overnight delivery (book only) - $15.89

Separate payment to Akron-Summit County Public Library (cash, credit, check or money order):

  • Execution fee per application - $25
  • Photo (optional) - $12

Passport Renewals

If you are renewing an expired adult passport issued within the last 15 years, you will have to apply by mail. Renewal forms are available online through the U.S. Department of State website, at the TechZone@Main or at the Nordonia Hills Branch Library.

Frequently Asked Questions

How long does it take to receive my passport?

Routine service takes approximately 4-6 weeks from the time of application. The Library can provide expedited service for an additional $60 fee, which reduces the wait time to 2-3 weeks.

Where can I get a copy of my birth certificate?

If you were born in Ohio, visit the state's Department of Health website. If you were born outside of Ohio, visit the CDC website for birth certificate information listed by state.

Where can I get information on the status of my application?

Check the status of your application on the U.S. Department of State website or call the National Passport Center at 1-877-487-2778.
